Well Woman Care to Gynecological Surgery

Gynecologist Boerne, TexasGynecology is an area of women's health that ranges from routine annual well-woman exams to highly specialized surgical procedures to treat specific disorders related to the uterus, ovaries, cervix, vulva, bladder, and vagina. Dr. Kocay has extensive experience and training in all areas of gynecology.

Some procedures are able to be performed in the office for your convenience. For all those requiring inpatient/outpatient hospitalization, arrangements will be made with the Methodist HealthCare System in San Antonio.

General Gynecology Services

Well-Woman Care

This exam should be performed annually. During this visit Dr. Kocay will perform a physical exam and general health screening. This includes a comprehensive exam, pap smear, various lab tests and mammography/colonoscopy/bone density testing if indicated. It is important to bring up any health concerns at this time. Preventive care is an important aspect of maintaining overall health.

Family Planning

Dr. Kocay will meet with each patient and discuss which option of family planning / contraception is right for them. Available methods are barrier methods (condoms diaphragms), hormonal methods (birth control pills), intrauterine devices (IUD), natural family planning (rhythm method) and permanent sterilization (bilateral tubal ligation and Essure®).

Pelvic Pain

Chronic pelvic pain is an often poorly understood condition many woman experience at some point in their lifetime. While some known etiologies include endometriosis, ovarian abnormalities and infection, some women suffer from chronic pain unnecessarily. If pain persists for more than a few days, an appointment should be made to consult with Dr. Kocay.

Abnormal Bleeding

Irregular menstrual periods can result from abnormal uterine bleeding. Contributing factors may include weight loss / gain, stress, sickness or medication side effects. To diagnose this condition, Dr. Kocay will obtain and review a patient’s medical history and perform a complete examination. Some medical tests he may use are ultrasound, hysteroscopy and /or laparoscopy. Sometimes an endometrial biopsy is necessary to rule out uterine cancer. Treatment for this condition includes medication, hormone therapy and /or surgery.

Abnormal Pap Smear

A pap smear is a screening test aimed at detecting potentially pre-cancerous changes and cancerous cells in the cervix. Positive finding are referred to as an abnormal pap smear. The test remains a very effective, widely used method for early detection of pre-cancer and cervical cancer. If a pap smear comes back abnormal, Dr. Kocay will discuss all treatment options available and provide guidance through each step of the process to recovery.

Menopause

This is a permanent cessation of ovarian function that occurs in women typically between the ages of 45 and 55. Many changes occur during menopause including hot flashes, bone deterioration, mood swings and decreased libido. Dr. Kocay will help the patient to understand these changes and assist in alleviating intolerable symptoms. The decision whether or not to begin hormone replacement therapy is one the patient and Dr. Kocay will make together after going over individual risk factors.

Gynecological Surgery

Hysterectomy

This is the most common gynecological surgery performed in the United States and an estimated one third of all U.S. women will have a hysterectomy by age 60. It is used to treat certain kinds of cancers, endometriosis, abnormal bleeding, chronic pain and fibroids. It involves removing the uterus and is performed in the hospital setting. Dr. Kocay is highly experienced with laparoscopic, vaginal, abdominal, as well as DaVinci robotic hysterectomies. He can discuss and explain which surgical option is the best treatment plan for you.

Laparoscopy

A laparoscopic surgery allows doctors to perform both minor and complex surgeries with a few small cuts in the abdomen with the aid of a camera. Dr. Kocay has vast experience with laparoscopy and uses it to inspect, diagnose and treat many different gynecological conditions. One benefit of this procedure is a decreased recovery time compared to traditional surgeries.

Hysteroscopy

This procedure involves looking at the inside of the uterus with an endoscope and can be done as an outpatient procedure with minimal discomfort. Dr. Kocay uses this procedure to treat fibroids or polyps that may be causing discomfort or bleeding.

Endometrial Ablation

This is a procedure that is used to remove (ablate) the lining of the uterus. This technique is most often employed for women who suffer from heavy vaginal bleeding but do not wish to undergo a hysterectomy. Dr. Kocay performs ablations as an outpatient and recovery time is minimal.

Tubal Ligation

Considered a permanent form of sterilization, this procedure involves severing or sealing the fallopian tubes to prevent fertilization. This procedure has a 99% rate of effectiveness in preventing pregnancy and is therefore a popular choice of family planning for women who are finished with child bearing. Dr. Kocay can perform this procedure in the hospital after delivery of a baby or as an outpatient procedure.

Essure®

This in-office procedure is the newest and most effective form of permanent birth control now available on the market with an effectiveness rate of 99.8%. This gentle, non-surgical procedure works with your body by creating a natural barrier against pregnancy and is performed without the risks of surgery or general anesthesia associated with tubal ligation. Performed in approximately 10 minutes in the comfort of our office, Essure requires minimal recovery time with little to no discomfort. Most women can return to normal activities the next day.

LEEP

Loop Electrosurgical Excision Procedure is one of the most commonly used approaches for treating cervical dysplasia and can be done under local anesthesia in the office with minimal discomfort. Dr. Kocay uses a device to remove and cauterize the areas of dysplasia on the cervix with minimal risks or complications.

Anterior/Posterior Repair

This surgical procedure repairs defects to the walls of the vagina. It is the surgical intervention used to treat cystocele (protrusion of the bladder into the vagina) and rectocele (protrusion of rectum into the vagina).

Bladder Suspension

This surgical procedure is used to treat urinary stress incontinence. Stress incontinence is a bothersome condition resulting in the loss of urine when sneezing, coughing or laughing. Dr. Kocay performs a highly effective sling procedure in an outpatient facility.
